Form 63 specifically says the temporary permit will not be issued until the licensure application has already been submitted and determined complete.

The current application was revised in October 2025 and contains several details that are easy to miss.
- Fully completed, typed Form 1. The Board warns that illegible applications can be returned for a new legible copy.
- $60 nonrefundable application fee. Check, money order or the Board’s listed credit-card/payment route is available.
- Legible Social Security card copy.
- Current U.S. government-issued photo ID. The form gives examples such as a driver’s license, state photo ID, military ID or qualifying resident card.
- Exact name matching. The name on the application, ID and Social Security card must match.
- Legal name-change documentation. Marriage, divorce or other qualifying court/government records may be needed when prior education or training uses another name.
- Training information. List each school, location, complete attendance dates and hours earned.
- High-school or GED verification. Follow the form’s instructions for records supplied by the issuing school/agency or accepted digital-delivery service.
- Citizenship/immigration status information. The current form asks the applicant to identify the applicable status and may require supporting immigration documentation.
- Felony and disciplinary disclosures. If applicable, include the required case numbers and Board disclosure forms rather than assuming an old record can be omitted.
Foreign professional training
Training completed outside the United States must be evaluated for equivalency. The Board says the credential evaluation must be sent directly to the Board and acceptance is considered case by case.
Foreign high-school education
Foreign secondary education also requires an educational-equivalency evaluation when used to satisfy the education requirement.
Kansas Cosmetology Exams in 2026: Two Computer-Based Written Tests
The current Cosmetologist Candidate Information Bulletin is the September 2025 revision. It requires both a written theory examination and a written practical examination.
